Trackmond is activated by hospitals on behalf of their patients. If you'd like to use the app, ask one of the participating hospitals below to enrol you — they handle the activation through their dashboard. There's no in-app subscription for patients to manage themselves.
If your hospital isn't on Trackmond yet, they can request to join through the Hospital Command Centre. We'll get back to them within 48 hours.
